Introduction
The characteristic boom lift fatality is not a fall. It is being crushed by the machine you are standing in, while it continues to run.
The sequence is consistent and it takes seconds. An operator is manoeuvring at height, looking upwards at the work rather than at the controls. The platform moves up, or forward and their head, neck or torso comes against a beam, a soffit, a pipe run or a section of steelwork above them. Their body is now pressed against the control lever, holding it in operation. The machine does exactly what it is being told and keeps driving into the obstruction. Nobody at ground level necessarily notices, because from below the platform simply looks stationary.
This is entrapment, and it is the reason boom-type mobile elevating work platforms are treated differently from scissor lifts. It is also why three arrangements matter more on a boom lift than almost anything else: secondary guarding devices fitted to the platform, somebody at ground level who knows how to operate the emergency lowering controls, and the simple rule that nobody operates a boom lift at height alone.

Two further points shape the course.
The first is that a harness in a boom lift is worn for restraint, not fall arrest and the distinction matters. Occupants are not principally at risk of climbing out and falling; they are at risk of being catapulted from the platform when a boom strikes something, a wheel drops into a hole or the machine is jolted, all of which whip the basket violently. A short restraint lanyard attached to the designated anchor point keeps the occupant in the platform. A long lanyard, or one intended for fall arrest, permits exactly the movement the restraint is meant to prevent, and can allow somebody to be thrown out and then suspended outside the basket.
The second is that boom lifts overturn because of the ground, not the wind. They carry a high centre of gravity on a relatively small footprint, and extending the boom moves that centre of gravity outwards. Soft ground, backfilled trenches, drain covers, kerbs, potholes, slopes and voids all produce overturns most commonly while the machine is being driven with the platform elevated, which many machines permit and which is the point at which they are least forgiving.

3. What is entrapment?
The crushing of an operator between the platform or its controls and a structure above or beside them. It typically happens while the operator is looking upwards at the work rather than at the controls, and the platform continues moving into a beam, soffit, pipe run or section of steelwork. The operator’s body is then pressed against the control lever, holding it in operation, so the machine keeps driving into the obstruction. It is the characteristic cause of death in boom-type platform work and it takes seconds.
4. What is sustained involuntary operation?
The mechanism that makes entrapment lethal rather than merely injurious. Most MEWP controls require deliberate pressure to operate and stop when released but a trapped operator’s own body weight can maintain that pressure. The machine is not malfunctioning; it is being operated, by a person who cannot stop operating it. This is why the presence of somebody at ground level who can shut the machine down is so important.
5. What is secondary guarding?
Devices fitted to the platform to interrupt operation if the operator is pushed against the controls. Designs vary pressure-sensitive bars in front of the control panel, hoops, proximity sensors and systems that cut or reverse movement when activated. They reduce the risk and do not eliminate it, and they do not remove the need for the operator to look up, plan the route and position themselves sensibly. A machine with secondary guarding still requires ground personnel.
6. Can a boom lift be operated alone at height?
It should not be. Somebody must be at ground level, trained in the emergency lowering controls, and present while the platform is elevated. An entrapped operator cannot help themselves, cannot call for help, and may not be visible as being in difficulty from below. A colleague who knows where the ground controls are and how to use them is the difference between an incident and a fatality. That person also serves for medical emergencies, machine faults and any situation where the operator cannot bring the platform down.
7. Where are the emergency lowering controls?
At ground level on every machine, and their exact location and operation vary by manufacturer and model. This is a familiarisation matter as much as a training one knowing that ground controls exist is training; knowing where they are on the machine on site today is familiarisation, and it should be established before the platform goes up rather than during an emergency.
8. Why do boom lifts overturn?
Almost always because of the ground. The machine carries a high centre of gravity on a relatively small footprint, and extending the boom moves that centre of gravity outward and away from the base. Soft or made ground, backfilled trenches, drain and inspection covers, basements and cellars, kerbs, potholes, slopes and voids all cause the wheels or outriggers to drop or sink. The most dangerous condition is travelling with the platform elevated, which many machines permit and which greatly reduces tolerance for uneven ground.
9. Should the machine be driven with the platform raised?
Only where the machine permits it, the ground has been assessed as suitable and there is no practical alternative and with particular care. Many overturns occur during elevated travel over ground that would have been unremarkable with the boom stowed. Lowering, repositioning and re-elevating takes longer and is considerably more forgiving, and where the route is uneven, sloping or unverified it is the correct choice.
10. What ground assessment is needed?
Enough to establish that the ground will support the machine’s loads without deforming. That means considering made ground, recent excavation and backfill, drain runs, culverts, basements and cellars, tanks, soakaways and mine workings, together with surface condition, gradient and edge proximity. Spreader plates distribute load where outriggers are fitted, and their size follows from the loading rather than from what happens to be available. Site drawings and service information inform the assessment.
11. Why is a harness worn in a boom lift?
For restraint to keep the occupant inside the platform. The principal risk is not climbing out but being catapulted: if the boom strikes a structure, a wheel drops into a hole or the machine is jolted, the platform can whip violently and eject an occupant. A harness with a short restraint lanyard attached to the designated anchor point prevents that.
12. What is the difference between restraint and fall arrest?
Restraint prevents the fall from happening. Fall arrest catches somebody after they have fallen. In a boom lift the correct arrangement is restraint, using a lanyard short enough that the occupant cannot be thrown clear of the platform. A long lanyard, or an energy-absorbing fall arrest lanyard, permits exactly the movement restraint is meant to prevent somebody can be catapulted out and then left suspended alongside the basket, which converts a preventable event into a suspension rescue.
13. Where should the lanyard be attached?
To the designated anchor point within the platform, identified by the manufacturer. Never to a handrail unless it is specifically designated, and never to an adjacent structure. Attaching to a building or steelwork in order to step across, or to remain connected while the machine moves, is a recognised cause of serious injury the machine can move away while the occupant is anchored to something that does not.
14. Can you climb out of the platform?
Only where this has been specifically planned, assessed and permitted, with arrangements for the transfer and for getting back. It is not routine. A boom lift is a work platform, not an access route to a structure, and unplanned transfers place the occupant between two things that can move relative to each other.
15. What load limits apply?
Total platform load including occupants, tools and materials, the maximum number of occupants, and frequently overlooked the maximum permitted side force, which is the horizontal force an occupant can apply from the platform. Pushing or pulling against a structure, dragging materials into position or reacting against a stuck component all generate side loads the machine is not designed to resist, and they can destabilise it.
16. What should a pre-use inspection cover?
Broadly: tyres and wheels, chassis and outriggers, boom and platform structure for damage or cracks, hydraulics for leaks and damage, controls at both platform and ground positions, emergency lowering function, secondary guarding devices, guardrails and gates, anchor points, restraint devices, tilt and interlock alarms, horn, lights and beacons, and the data plate and load information. Emergency lowering should be verified as functional before the platform is used, not assumed.
17. What is thorough examination?
A statutory examination by a competent person at intervals set by legislation, with a report issued. MEWPs are lifting equipment used for lifting people, which places them in the more frequent examination category. It is separate from servicing and from the operator’s daily check, and the report should be available for the machine in use.
18. What is the risk from overhead power lines?
Severe, and contact is not required. A boom lift is a large conductive structure that can be manoeuvred close to high voltage lines, and electricity can arc across an air gap to the boom or platform. The operator, the machine and anybody touching it at ground level are at risk. Controls include establishing whether lines can be isolated or diverted, defining exclusion distances, physical barriers and goal posts, spotters, and planning the machine’s working envelope so that approach is impossible rather than merely discouraged.
19. What wind limits apply?
Those specified by the manufacturer for the machine, and they are limits rather than guidance. Wind speed increases with height, so conditions at the platform are worse than at ground level. Large or flat items being handled sheets, panels, signage, cladding act as sails and generate forces well beyond what their weight suggests. Machines also have out-of-service requirements when left unattended in exposed conditions.
20. What is familiarisation and why does it matter?
Instruction on the specific machine model being used, covering its controls, emergency systems, load and range limitations and any features particular to it. Category training establishes that somebody can operate that type of machine; familiarisation establishes that they can operate this one. Controls, emergency lowering arrangements and secondary guarding differ substantially between manufacturers, and an operator who has never seen this model before is not ready to take it up. Familiarisation is the employer’s or hirer’s responsibility and should be recorded.
21. Does this Award authorise me to operate at work?
No. Certification is category-specific and evidences training and testing. The employer remains responsible for familiarisation on the particular machine and for authorising the individual to operate. A boom lift certificate does not cover scissor lifts or static machines, which are separate categories.
22. Are there any entry requirements?
