9. How is a service actually located?
By combining sources. Plans indicate what may be present. Locating equipment cable avoidance tools and signal generators detects services in the ground, and the person using it needs to be trained, because these devices have significant limitations and can miss unenergised, non-metallic or deep services. Trial holes dug by hand confirm the actual position and depth. Marking on the ground records what has been found, and it needs refreshing as work proceeds.

10. What is safe digging practice?
Locating before digging, marking what has been found, and using hand digging or approved non-mechanical methods in the vicinity of known or suspected services. Mechanical excavation is kept away from marked services by a defined margin, and hand digging proceeds carefully with tools used in a way that does not spike a cable. Exposed services are supported and protected rather than left hanging in a trench, and they are not used as steps, supports or handholds.

11. What should happen if a service is struck or exposed?
Stop work immediately and keep everybody clear. For an electrical strike, nobody touches the machine or the ground around it, the operator remains in the cab unless there is fire, and the distribution network operator and emergency services are contacted. For gas, evacuate upwind, eliminate ignition sources, do not operate switches or vehicles nearby, and contact the emergency gas service. For water, isolate if possible and consider undermining and flooding. In every case the strike is reported, whether or not damage appears significant a nicked cable that continues working can fail catastrophically later.

12. Why does excavation collapse kill so quickly?
Because of weight and speed. A cubic metre of soil weighs over a tonne, collapse happens in a moment with no warning, and a person buried even to chest height cannot expand their chest against the pressure and suffocates. Rescue is extremely difficult because the ground that collapsed once will collapse again onto rescuers. This is why prevention is the entire strategy.

13. What makes ground unstable?
More than most people expect. Soil type and water content, rainfall and groundwater, drying and shrinkage, vibration from plant, traffic or piling, previously disturbed or made ground, adjacent loads including spoil, plant and structures, weather changes and simply time. Ground that stood unsupported all morning is not evidence that it will stand this afternoon, and the assumption that it will is behind a great many burials.

14. How are excavations supported?
By battering the sides back to a safe angle, by benching in steps, or by installing support timbering, proprietary shoring, hydraulic props or trench boxes. The choice depends on ground conditions, depth, adjacent loads and duration, and it is a decision for someone competent rather than a judgement made at the edge. Support is installed as the excavation proceeds and removed in a controlled sequence, and it is never installed by somebody standing in an unsupported trench.

No, on the basis that it looked fine. Entry into an excavation that has not been assessed and, where required, supported or battered is how people are buried. 16. Where should spoil be placed?
Well back from the edge. Spoil stacked at the edge loads the ground precisely where it is weakest and is a direct cause of collapse the excavation is undermined by its own arisings. The same applies to plant, materials, vehicles and site accommodation positioned near an edge. Distance from the edge is a control, not tidiness.

17. What inspection do excavations require?
Regular inspection by a competent person, at defined points including before each shift, after any event likely to have affected stability such as heavy rain or an accidental fall of material, and after any substantial change. Findings are recorded, and work does not proceed where an inspection identifies a problem until it has been resolved.

18. What is the risk with quick hitches?
Attachment release. Semi-automatic quick hitches require a safety pin or locking device to be fitted and correctly engaged, and where it is not, the bucket can detach during operation and fall onto whoever is below or in front.
