A great many organisations have a rescue kit. It is in a bag, in a van, or on a shelf in the store. It was bought when somebody asked whether there was a rescue plan, it has a certificate, and it has been inspected. What frequently does not exist is anybody currently on site who has used it, trained with it within any recent period, or practised recovering an unconscious person of average weight from an anchor point that is not conveniently placed. A team that believes it can perform a rescue, and cannot, is in a considerably worse position than a team that knows it cannot because the first will let work proceed.

A note on scope, which should be read before purchasing. Rescue is a physical skill. Three points shape the course.

The first is that fall arrest converts one emergency into another. A person whose fall has been arrested is alive, which is the system working, and is now suspended in a harness with a clock running. Suspension restricts circulation, and a casualty who is unconscious, injured or unable to move their legs deteriorates considerably faster than one who can shift position and use relief straps. The rescue therefore has to be measured in minutes, which removes the emergency services from the plan in most situations.

The second is the rescue hierarchy, which mirrors the hierarchy for the work itself. Self-rescue the person recovering themselves is fastest and safest. Assisted self-rescue comes next. Team rescue by trained colleagues follows. Emergency service rescue is last, and in many situations is not a realistic option at all, particularly on remote sites, at significant height, inside structures, or where specialist rope teams would have to be mobilised.

The third is that plans have to be tested against the actual site. Organisations using fall arrest systems benefit from both, and if only one is taken, the broader Award should come first, since preventing falls matters more than recovering from them.

4. Why does an arrested fall create a second emergency?
Because the person is now suspended. The fall arrest system has done its job they are alive, and that is the point of it but they are hanging in a harness, possibly injured, possibly unconscious, possibly against a structure, and their condition will not remain stable. Suspension restricts circulation, and the harness concentrates that restriction at the thighs and groin. A casualty who cannot move deteriorates faster than one who can shift position and use relief straps, and an unconscious casualty cannot do either.

5. How long is available?
Less than people assume, and the honest answer is that it varies with the individual, their injuries, their consciousness and their position. The planning position is that rescue must be achievable in minutes rather than tens of minutes, which is what removes the emergency services from most plans. Precise timings should not be relied upon as a target the arrangement should recover the person as quickly as it can be made to, not within some notional limit.

6. Should a suspended person do anything while waiting?
If they are conscious and able to, yes. Moving the legs, pushing against a structure or a step if one is reachable, and using relief or trauma straps if fitted all help maintain circulation. Relief straps are a simple addition to a harness that allow the wearer to stand in a loop and take pressure off the leg straps, and they cost very little. None of this assists an unconscious casualty, which is why the plan cannot depend on it.

7. What is the rescue hierarchy?
Self-rescue first the person recovers themselves, which is fastest and involves nobody else being exposed. Planning to call 999 is planning to hope.

9. What rescue methods exist?
Broadly, methods that lower a casualty to a safe level, methods that raise them, and methods that involve a rescuer reaching them. Descent devices allow a controlled lower from an anchor. Rescue kits provide a haul and lower capability, frequently with mechanical advantage so that one person can move a casualty’s weight. Pick-off techniques involve a rescuer reaching a suspended casualty and taking their weight to lower both. Which is appropriate depends entirely on the structure, the anchor points, the clearance and the training the team holds and selecting a method is a decision made when planning, not while somebody is hanging.

10. What about rescue from a powered access platform?
It is a different problem and frequently a simpler one, provided somebody has thought about it. Every machine has ground-level emergency lowering controls, and the essential requirement is that a person at ground level knows where they are, how to operate them and is available while the platform is in use. That single arrangement resolves a large proportion of powered access emergencies a medical event in the basket, a machine fault, an operator unable to bring themselves down.

11. What is entrapment and why does it complicate rescue?
On boom-type machines, an operator can be crushed between the platform and a structure above or beside them, frequently after inadvertently operating the controls while looking upwards. It is one of the recognised causes of death in powered access work. It complicates rescue because the casualty may be pinned against the very controls needed to move the platform, and because releasing the crushing force may worsen their condition. 12. What about ladders and towers?
Both present recovery difficulties out of proportion to their height. A casualty on or beside a ladder is in a confined vertical space with limited room for a rescuer to work. 13. Do rescue anchors differ from work anchors?
They can. An anchor adequate for arresting one person’s fall may not be appropriate for a rescue involving two people and dynamic loading, and it may not be in the right place an anchor perfectly positioned for work may be unreachable or unusable for a lower. Rescue anchor provision is part of planning the work, not something to be identified afterwards.

14. How does clearance affect rescue?

15. How many people does a rescue need?
More than one, almost always. A plan that depends entirely on one person is a plan that fails if that person is the casualty, and lone working at height with fall arrest is a situation requiring particularly careful thought.

16. How is a rescue plan tested?
By walking it through in the actual location with the actual equipment, before work begins. That reveals the anchor that is out of reach, the doorway the stretcher will not pass through, the radio that does not work inside the structure, the equipment that is locked in a van two hundred metres away, and the assumption that the casualty weighs the same as the lightest member of the team. 17. What happens to equipment after a fall?
It is withdrawn from service. If nobody can say who will perform the rescue, with what equipment, within what time, using which anchor, to what clearance the arrangement does not exist, and the work at height should not proceed on personal fall protection. The alternative is to eliminate the need, use collective protection, or use work restraint so that a fall cannot occur.
